Rajasthan RGHS Update: Up to ₹30 Lakh Package Approved for CAR-T Cell Therapy, Pensioners Also Covered

The Rajasthan government has announced an important healthcare measure for eligible government employees, pensioners and other beneficiaries battling certain serious blood cancers. In a move that could significantly reduce the financial burden associated with advanced cancer treatment, CAR-T Cell Therapy has been brought under the approved treatment package of the Rajasthan Government Health Scheme (RGHS).

The decision, taken under the initiative of Chief Minister Bhajanlal Sharma, provides financial coverage of up to ₹30 lakh for eligible patients undergoing CAR-T Cell Therapy.

The treatment is among the newer forms of cancer immunotherapy and can be extremely expensive. Bringing it within the government health scheme could make the treatment financially accessible to eligible patients who meet the required medical criteria.

Here is what beneficiaries need to know about the new package, its coverage and how CAR-T Cell Therapy works.

RGHS to Provide Up to ₹30 Lakh for CAR-T Cell Therapy

Under the approved arrangement, the total package for CAR-T Cell Therapy can go up to ₹30 lakh.

Of this amount, approximately ₹26 lakh has been earmarked for the manufacturing of CAR-T cells, which is one of the most specialised and expensive stages of the treatment.

Another ₹4 lakh can be used for supportive treatment and management of complications associated with the therapy.

The financial package is expected to reduce the out-of-pocket burden on eligible patients and their families when undergoing this highly specialised form of cancer treatment.

What Will the ₹4 Lakh Supportive Care Component Cover?

CAR-T therapy involves more than the preparation and infusion of modified immune cells. Patients need close medical monitoring because certain complications can occur during or after treatment.

The supportive care component of the package can cover prescribed requirements associated with the treatment, including medicines such as Tocilizumab and IVIG, hospitalisation and management of potential complications.

The package also provides for applicable medicines for up to 30 days after discharge, according to the prescribed treatment provisions.

This is significant because medical monitoring and supportive care following CAR-T treatment are important parts of the overall therapy process.

Rajasthan Government Pensioners Can Also Get the Benefit

The benefit is not restricted only to serving employees covered under RGHS.

Pensioners receiving medical facilities under the Rajasthan State Pensioners Medical Concession Scheme, 2021 have also been brought within the scope of this treatment facility, subject to the applicable eligibility requirements.

Eligible beneficiaries receiving healthcare facilities through RGHS under the RCS (MA) Rules, 2013 are also included under the arrangement.

The decision therefore extends access to a wider group of eligible government healthcare beneficiaries rather than limiting the advanced cancer treatment package to serving employees alone.

What Exactly Is CAR-T Cell Therapy?

CAR-T Cell Therapy, or Chimeric Antigen Receptor T-cell Therapy, is an advanced form of immunotherapy designed to use a patient's own immune system to fight cancer.

The treatment begins by collecting specific immune cells known as T-lymphocytes or T cells from the patient's blood.

These cells naturally play an important role in the body's immune response. However, in CAR-T therapy, the collected T cells undergo specialised modification in a laboratory.

They are engineered so that they can recognise particular targets found on cancer cells.

After the cells have been modified, they undergo required quality and safety checks. The prepared CAR-T cells are then infused back into the patient's body.

Once inside the body, these engineered immune cells are intended to identify and attack targeted cancer cells.

Why Is CAR-T Therapy Considered a Specialised Treatment?

Unlike conventional medicines that can be manufactured in large quantities and prescribed to many patients, CAR-T treatment involves preparing modified immune cells for an individual patient.

This personalised process requires sophisticated laboratories, specialised medical teams and strict quality-control procedures.

Patients also need careful monitoring before, during and after treatment because CAR-T therapy can be associated with potentially serious side effects.

This combination of personalised cell manufacturing, hospital care and post-treatment monitoring is one reason the overall cost of the therapy can be substantial.

Is CAR-T Cell Therapy Available for Every Cancer Patient?

No. The inclusion of CAR-T therapy under RGHS does not mean that every person diagnosed with cancer will automatically qualify for the ₹30 lakh treatment package.

CAR-T therapy is used for selected medical indications and is not considered suitable for all forms of cancer or every patient.

Eligibility will need to be determined by the appropriate specialists or expert medical committee.

Several factors can be considered before recommending treatment, including the type of cancer, stage and characteristics of the disease, treatments already received and the patient's overall health condition.

The patient will also need to meet the prescribed medical criteria.

Medical Evaluation and Pre-Approval Will Be Important

Eligible beneficiaries should not assume that they can directly undergo CAR-T therapy and subsequently claim the entire amount under RGHS.

The prescribed approval process must be followed.

Patients may need to undergo relevant diagnostic tests and evaluation by specialists. An expert committee may then examine whether CAR-T therapy is medically appropriate in the individual case.

Where required, the committee's recommendation and pre-authorisation or pre-approval will have to be obtained before treatment begins.

Beneficiaries should therefore check the latest RGHS procedure and obtain the necessary approvals before proceeding with treatment.

Major Financial Relief for Eligible Families

Advanced cancer treatment can place considerable financial pressure on families, particularly when specialised therapies are required. By approving a package of up to ₹30 lakh for CAR-T Cell Therapy, the Rajasthan government has expanded the range of high-cost treatments available to eligible beneficiaries through its healthcare framework.

For government employees, pensioners and other covered beneficiaries dealing with eligible blood cancers, the decision could provide substantial financial assistance when CAR-T therapy is recommended by medical specialists.

Patients and families considering the treatment should consult their treating oncologist and verify the latest eligibility, empanelled hospital, documentation and pre-authorisation requirements under RGHS before starting the procedure.
